Born May 17, 1949, In West Virginia, He left us quietly on August 8th 2011. At the Hospice House of Sarasota.
He is survived by his wife Kathryn Parks-Smith.
His Three Little Birdies: Alisa, Summer, and April.
Four grandchildren: Samantha, Lee, Cameron, and Aerial.
Five brothers and sisters: Elwood, Alethea, Peggy, Buddy, and Linda.
Services for his racing community are announced on Karnac.com as well as other media ports.
Jack’s family and friends may remember him together on August 21, 2011 at 2:00pm at Alan Moore Funeral Home in Ellenton. 1222 Ellenton-Gillette Rd.
